[SRFD3 detected a power supply fault] Check coolant flow/temperature.
Check inside of Cabinet is not Over temperature. If coolant flow/temperature
and cabinet temperature OK and the fault persists, may need to replace
SRFD3. |

[SRFD3 detected a peak current overload condition in one or
more RF devices] Check that system center frequncy consists of 8 digits
and is correct. Check RF Amp error(s) that may point to the real cause
don't precede this error(s). If so, then invetigate preceeding
error(s) first. Check Exciter RF Output and Cable between SRFD3 and
Exciter. If fault persists, replace SRFD3. |

[SRFD3 detected an RF device shorted fault] SRFD3 will attempt
to bypass this failure and run in low power mode. If the bypass is
successful, message number 2254054 will appear. Order an RF Amplifier
and replace the bypassed unit as soon as the new one arrives. The
customer can scan with this configuration unitl the new RF Amplifer
arrives. Bypassing the failed FET is NOT a parmanent fix. The penalty
for bypassing an output FET is about 6.25% reduction in maximum RF
output power. If the FET failure cannnot bypassed, the SRFD3 must
be replaced immediately. The site will not be able to scan. |

[SRFD3 detected High Voltage Low fault] Power cycle the SRFD3
so that the power is off for 3 minutes and then attempt to Scan. Check
incoming AC line phases to SRFD3. If fault persists, replace SRFD3.
(It is difficult to know how long the unit will run. You may want
to order an amplifier in case the unit fails within the next fer days.) |

[SRFD3 detected High Voltage High fault] Power cycle the SRFD3
so that the power is off for 3 minutes and then attempt to Scan. If
fault persists, replace SRFD3. (It is difficult to know how long the
unit will run. You may want to order an amplifier in case the unit

flow/temperature. Check inside of Cabinet is not Over temperature.
If coolant flow/temperature and cabinet temperature OK and the fault
persists, may need to replace SRFD3. (Additional time may be required
to allow the system to cool, e.g. 15-20 minutes.) |

[SRFD3 detected Peak Power fault] Perform BODY AND HEAD MAXIMUM
POWER RF OUTPUT CHECK. Check Exciter RF output and RF Cables. If the
problem persists, may need to replace SRFD3. |

ocurrs with other standard PSDs. (Usually this fault happens because
the scan PSD is exceeding the limits of the amplifier using reserch
mode or customer built PSD). Perform BODY AND HEAD MAXIMUM POWER RF
OUTPUT CHECK. Check Exciter RF output. If pulse sequences are acceptable,
and the fault persists, may need to replace SRFD3. |

[SRFD3 detected a reflected power fault] Visually check all
connections in Body or Head transmission lines for damage from the
front of SRFD3 to where the connections are made to the RF coil. Perform
BODY AND HEAD MAXIMUM POWER RF OUTPUT CHECK. If all connections are
good and the Maximum Power RF Output check is passed, and the problem
persists, may need to replace SRFD3 or contact OLC for support. |

[SRFD3 detected an RF device junction temperature fault] Make
certain that this error isn't being preceeded by another in the
error log. If another RF error preceeds it, troubleshoot the other
error first. Check coolant flow/temperature. Check inside of Cabinet
is not Over temperature. Confirm that the system center frequency
consists of 8 digits and is correct. If other potential sources have
been eliminated and the fault persists, replace SRFD3. |

SRFD3 J20 and ASC Lite. Replace cable if defective. If problem persists,
may need to replace ASC-Lite or SRFD3. |

[SRFD3 was shutdown by the UPM due to RF safety limits being
exceeded] Possible UPM fault or system issue. Check patient weight
value entered by customer for scan and make sure it has not been significantly
under-reported. Check TG from customer scans and make sure if it has
not invresed from what it is normally due to a system hardware problem.
Investigate the cause of any substantial increase in TG. Check Config
File Mgr and make sure Start TA, Coil Loss and Cable Loss values for
the coil in use when the fault happens are correct. Check the cable
connections between SRFD3 and ASC Lite. Perform UPM functional check.
If needed, perform UPM Power Monitor Calibration. If fault persists,
contact OLC for support |

[SRFD3 in low power state] SRFD3 has the ability to bypass
some types of RF FET failures in the event that one should occur.
If this message appears in the log, check to make sure that it was
preceded by Error Message Number 2254587, which indicates a true RF
FET failure. The FET failure may have occurred at some point prior
to the user first noticing this message about the low power state,
and the FET failure message will log only once. This error message
will appear every time a TPS reset is performed as a reminder to the
operator that scanning is limited until the amplifier is replaced.
The ultimate solution is that SRFD3 must be replaced. The low power
state allows the site to continue scanning at reduced output until
a replacement is installed. |
